This is a comprehensive psycho-educational evaluation to assess an individual's cognitive, academic skill and ability, as well as diagnosing various emotional, social, and behavioural issues. This assessment is suitable for issues relating to Giftedness, report for school transfer, Dyslexia and learning disabilities, Autism, Aspergers, Attention-Deficit Hyperactivity, Anxiety, Depression, Anger Control, Alcohol Abuse, etc.

IQ & Cognitive Test
  • Woodcock-Johnson Test of Cognition - Third Edition (WJ-III) (Equivalent to WISC/ WAIS)~ Age 2 to 90+

Academic Achievement Test
  • Woodcock-Johnson Test of Achievement - Third Edition (WJ-III) (Equivalent to WIAT)~ Age 2 to 90+

Behavioral/ Emotional/ Social Test
  • Adaptive Behaviour Assessment System (ABAS) ~ Age 0 to 89
  • Behavioral Assessment Scale for Children - Third Edition (BASC-3) ~ Age 2 to 25
  • Conners' Rating Scales - Revised (L) (CPRS-R:L) ~ Age 3 to 17

Cost

  • Whole Package (IQ, Achievement, Adaptive [social] skills & Behavioral): $2000
  • Behavioral only: $850 (For those who had already done cognitive and achievement assessment no more than 1 year ago)

Procedure

  1. Client/Parent emails The Counselling Place office, providing information on age and gender of child/teen, and issues of concerns/reasons for referral.  The Counselling Place administrative staff would assist in setting up appointments and answering general enquiries. Any specific questions you may have, please reserve it for the Psychologist who you will meet in session. 
  2. An initial 4-hour appointment is set up.  At least one parent/caregiver (ideally both parents) responsible for primary care of child/teen will need to be present for this appointment with the child/teen.  The Psychologist would interview the parent/caregiver, determined appropriate tools for assessment, and answer any specific questions.  If at this stage it is determined that no assessment is required, appropriate referral/strategies would be given and the assessment process will terminate.  The client is charged regular counselling fee for an hour's consultation session.
  3. If it is determined that assessment is required, Psychologist would proceed with assessment of the child/adolescent, which include objective (Psychologist’s evaluation) and subjective (parents’/caregiver’s, teacher’s information, child's/teen's) components.  Child/teen would be given a variety of verbal, physical, and written tasks.  Parents/Caregivers are encouraged to prepare the child/teen for assessment by informing the child/teen of the purpose and forms of the assessment.  Due to the confidential nature of the testing material, all tests mailed to third parties (e.g., teachers) will be sent by courier. Third parties are only contacted with permission of the parent/caregiver.
  4. Depending on the progress of the child/teen in completing the required tasks, subsequent appointment(s) may be set up in which only the child/teen needs to be present. 
  5. The Psychologist would start report preparation as soon as the child/teen complete all required tasks and all information required (from parents/caregiver or teacher) is received.  This will take about 2 weeks. 
  6. Final appointment set up with parent/caregiver to meet with the Psychologist to receive and discuss report.  One original and one copy of the report will be given.  Additional original report can be obtained at $50 each.
  7. Reevaluation using the same instrument can only be done in 6 months’ time to prevent practice effect.
